1) First month on the job at a bank processing center, I made the bank miss the nightly deposit, meaning they had to borrow money to cover outstanding transactions. It cost them about $35K.
2) First time in the cleanroom at current job, I'm trying to take pictures of a wafer and dropped the camera on the wafer, destroying it. Estimated cost about $100K.

Didn't get fired or even disciplined for either one. But learned.
