With the caveat that this is one person's account of the situation: David recommended a code module which was irreparable be rewritten (70% of other code had already been fixed). He was asked by Terry (senior manager?) to take over as dev manager - but stipulated he would only if this module rewrite was approved. Terry refused, David was sidelined, Terry hired a friend...
Cue breaking point.
