@lizardsquid I don't know what you're specifically thinking of, but looking at electrical engineering I think it's because too many people remain using stuff to deprecate or fix things:
I really don't know why there's never been an agreement to swap the negative and positive notation stuff; It's been known for decades, maybe a century.
The MOS in MOSFET referred to the materials they were made of. Often no longer correct either.

@lizardsquid I hear you. I spent all of the past two days debugging C++ code that was extending Node.js modules just so we could use a kernel interface that was introduced in the early 2000's for a feature which has been standard since the late 90's.
